Notice of License Form

A DSP that would like to obtain the Blanket License must submit a Notice of License (NoL) to The MLC. Click here to gain access to the form. 

Notice of Nonblanket Activity Form 

A DSP that would like to forego the Blanket License and is a Significant Nonblanket Licensee must submit a Notice of Nonblanket Activity (NoNA) to The MLC. Click here to gain access to the form. 

Service Configurations Form (Stand-alone)

Before reporting usages to The MLC, DSPs are required to complete this form, either as part of the Notice of License Form or using this stand-alone form. This form ensures that The MLC applies the correct compulsory licensing rates to the DSP's services. Click here

Certified Minimum Fee Disclosure Form (Standalone) 

DSPs are required to send their Certified Minimum Fee Disclosure (CMFD) before February 15. This can be done either as part of the Notice of License Form or the Notice of Nonblanket Activity Form. This standalone form should be used if the DSP decided to provide the CMFD after submission of a notice. Click here
